Tabor Village excels in patient safety and quality care, experts confirm

ABBOTSFORD, BC CANADA May 9, 2018 – Tabor Village has been ranked in the top per cents of all Canadian aged care organizations in delivering excellence in care, patient safety and operations by Accreditation Canada, a national leading independent surveyor of health-care organizations.

 

Following a recent and intensive on-site review, where surveyors spent three days examining all aspects of Tabor Village’s care and operations, the organization received Accreditation Canada’s highest honour – Exemplary Standing – having fulfilled 100 per cent of the required organizational practices.

 

In addition, Tabor Village met an exceptional 97.25 per cent of the designated accreditation standards set out by the surveyors – a tremendous achievement, not only in the field of aging, but within the Canadian health care sector.

 

“Residents and family members speak highly of Tabor Village.  They are happy to call Tabor home and feel that care is excellent and their loved ones are safe and well provided for,” wrote the surveyors in the Accreditation Report.  “The staff are skilled, and all live the mission, vision, and values of “caring from the heart.”

 

The Accreditation report states, “In addition to this top tier ranking, several exemplary initiatives magnify Tabor Village’s clinical competency and leading practices in care.  Staff are seen as experts in quality initiatives. Integrating a palliative approach emphasizing quality of life in residential care, which has enjoyed both spread and sustainability at Tabor and in the Fraser Health Authority.  Commendation noted the effort to recruit a significant number of volunteers enhancing quality of life.  Spirituality is central to the life at Tabor and is ably supported by the chaplain as well as the faith-based supports in the community”.

 

“Our Accreditation results clearly show that Tabor Village’s dedication to performance excellence is rooted strongly into organizational culture,” says Dan Levitt, Executive Director of Tabor Village.  “We have worked in partnership with our community to offer high quality care, specialized services and innovative programs focused on aging.”

 

In addition to the Accreditation Canada top rating, Tabor Court Assisted Living recently received a third consecutive Seal of Approval award by the British Columbia Senior Living Association.  Tabor Court consistently receives high satisfaction ratings from tenants who enjoy deliciously prepared traditional Mennonite meals, individualized clinical services, therapeutic programming and a well-appointed living environment.

Joanna Martin, Director, Resident Care highlighted three key exemplary achievements noted in the Accreditation report:

 

  • In the area of patient flow Tabor Village has implemented an exemplary innovative monitoring program in partnership with Fraser Health to identify the early warning signs of changes to senior’s health conditions.  Detected early the assessment allows for interventions to be acted on at Tabor Village resulting in a significant reduction in transfer to acute care.
  • A just culture is evident in the management of medication-related errors.  When errors are identified they are treated as learning opportunities where appropriate.  Education and related action plans are developed and documented with the involvement of staff, and errors are discussed to review processes and avoid reoccurrence.  There is also a full disclosure to the resident and family when an incident occurs.
  • Remarkable care is being delivered as evidenced by high satisfaction ratings.   Of note is the diversity and complexity of the senior population being served.  There is outstanding clinical competency and range of service.  Much work has gone into PointClickCare to enhance electronic health records, point of care technology, electronic medication administration records, and clinical quality indicators.

 

In the Accreditation report, the surveyors specifically highlighted:

 

  • Tabor Village is blessed to have a highly dedicated, knowledgeable, and committed board of directors.
  • Visionary leaders seek to change the culture and narrative of dementia care, and work to create and adopt opportunities for innovation.
  • There is a strong caring partnership among leadership, direct service staff, seniors, and families around care programming that enhances quality of life.
  • A plethora of communication methods are used including meetings, huddles, newsletters, social media, handbooks, bulletin boards, memos, PointClickCare home page, Surge Learning and email.
  • An innovative group interview process ensures the efficient and effective recruitment and an excellent onboarding program supports new staff.
  • There is outstanding clinical competency and range of service advanced by clinical quality indicators.
  • Commendation was received for attracting and retaining a large and diverse group of volunteers who enhance and enable many programs and services.

 

Located in Abbotsford, Tabor Village operates two campuses serving nearly 300 people supported by 500 staff and volunteers.  As a teaching organization with affiliation agreements with local postsecondary institutions, Tabor Village is recognized locally and globally as a leading operator providing innovations in aged care and housing.
